Data and Notes:
Client details and working notes are kept in accordance with the BACP Ethical Framework for Good Practice in Counselling and Psychotherapy, The General Hypnotherapy Standards Council (GHSC), The General Data Protection Regulation (GDPR) (Regulation (EU) 2016/679 and Balens Health Professionals Policy.

Details will be kept for a period of seven years after the close of therapy and in the case of children and young adults for seven years following their legal age of majority. All information is held in a locked fireproof cabinet until it is securely destroyed on the destruction date, when your notes and any additional information I hold will be securely destroyed.

All therapy notes are kept anonymously and away from identifiable and signed named consent and contract forms.

Disclosures:
Confidentiality is of utmost importance, however, if I believe that you are in danger of harming yourself or others, or that a child is at risk, I reserve the right to inform outside agencies - I would, wherever possible, discuss this with you first.

If you disclose information about terrorist or criminal offences, I have a professional obligation to tell outside agencies.

Information may also be shared during professional supervision, of course client anonymity will be maintained.

Therapeutic Contract:
Full confidentiality details are stated within my therapeutic, or online therapeutic contract, and discussed during the initial therapy assessment.
